Our Care Management Team

Lara Furletti – Managing Director/Registered Nurse

Lara Furletti is a Registered Nurse with over 18 years’ experience in various healthcare settings, including aged care facilities, hospital wards, and an operating theatre where she worked as a Clinical Nurse Specialist. Lara has also served as a disability support officer and support worker, and is passionate about helping people achieve quality of life.

Lara also brings a wealth of knowledge to the Home Caring team, with an Advanced Diploma in Disability Work and Community Sector Management, and a Graduate Diploma and Certificate in Perioperative Nursing. She has a strong foundation in both clinical and managerial aspects of care, and works collaboratively to achieve great outcomes for her clients.

Rey Garcia – Case Manager/Registered Nurse

Rey Garcia is a Registered Nurse who has worked for 25 years as an anaesthetics nurse, recovery nurse, and an instrument and circulating nurse. He has also served as a paramedic in Kuwait, providing emergency response services for the community.

Rey is adept at working with multidisciplinary teams to deliver the best outcomes for patients. He believes supporting someone with a disability, or the older members of our community, is the most rewarding career you can have.

Don Fontabla – Case Manager

Don Fontabla has worked in the aged care industry for over six years, and throughout his time has held a wide range of roles in human resources, accounting, and administration. He also has extensive experience in customer service as a national sales and territorial manager, sales and marketing manager and purchasing officer.

With two Masters in Business Management and Leadership respectively, and a Bachelor of Arts specialising in Communications, Don enjoys collaborating with others, working alongside clients and their families to deliver great care that meets their needs.

Shannyn Gereg – NDIS Case Manager / Psychosocial Recovery Coach / Support Coordinator

Shannyn Gereg helps people of all backgrounds achieve their goals and enjoy quality of life on their terms. With an extensive background in disability care and psychosocial recovery services, she has worked as an Occupational Therapy Assistant, a qualified Disability Development Support Officer, and a Residential Area Manager at a disability services organisation.

Shannyn holds qualifications in community services, (Disability/Residential/ATSS), a Bachelor of Applied Science in Psychology and Intellectual Disabilities, and a Diploma of Business in Community Services and Health Management. She is dedicated to ensuring clients receive high quality care that makes a real difference in their lives.

Reynaldo Ng Limsiaco Jr – Case Manager / Registered Nurse

Reynaldo is a Registered Nurse with over 26 years’ experience providing high quality care. He has worked as an Associate Nurse Unit Manager and Active Nurse Unit Manager in an operating theatre, and cared for patients in ICU and day surgery.

Reynaldo is an asset to the Home Caring team, using his experience and knowledge to help our clients thrive. He believes everyone deserves quality care that helps them live at home for as long as possible.

Jose Andrew Suarez – Case Manager / Registered Nurse

Andrew is a highly skilled registered nurse with over 16 years of experience in various healthcare settings. He is especially skilled at providing Dementia and Alzheimer care, providing clinical support for the elderly, falls prevention, and caring for people with respiratory conditions.

Andrew responds quickly to his client’s changing conditions, and is a great communicator who works with clients to promote their health and wellbeing. He enjoys helping people experience quality of life, and using his skills to make a positive impact on their lives.
